Notice for random pool list updates - 2nd Quarter
**If you have already sent in your updated participant list, THANK YOU! **
**It is not necessary to re-send your list.**
We are about to begin pulling randoms for the 2nd quarter of 2019. If you have not yet completed your previous quarter testing, it is urgent you contact Customer Service today at 800-295-2587 or email them at firstname.lastname@example.org to set up a clinic to complete your testing. At this time appointments are extremely limited. All testing MUST BE COMPLETED by March 31st, 2019. In order to best serve you, we are requesting your current updated employee list be sent in immediately so that we can begin preparing your 2nd quarter selections. If you have no changes this quarter, please send an email message simply stating “No changes” with your company name in the body of the message to email@example.com. If you do need to send in your list, below are some brief instructions for those who are new to this process.
Your list should include (1) employee name (first and last name), (2) occupation (are they CDL or NON-CDL), (3) home base (if applicable). Home bases are only used for companies that have employees located in multiple locations. Last but not least we will also need (4) a unique identifier for each driver. This unique identifier can be a social security number, driver’s license number, or employee identification number.
For expedient turnaround time, we ask that your employee list is sent in a spreadsheet format using the 4 items listed above. Microsoft Excel is most commonly used to create spreadsheets. Please note that lists not sent in Excel format will add extra processing time as we have to manually create the file.
Please email your list to firstname.lastname@example.org. You may also fax your list to 317-248-1072.
Please remember that it is your responsibility to ensure that clinics use the proper Chain of Custody (COC) forms for ALL drug testing; whether it is pre-employment, random, or post-accident testing. It is also your responsibility to notify us of any personnel changes. You must notify us any time an employee has been terminated, on leave of absence, or when you hire a new employee. Without this information being updated, we cannot ensure your compliance!
We would also like to take this time to let all of our Drug and Alcohol Program clients know that we will be utilizing Lab Corp testing locations moving forward as another source of clinics for our clients. Please let the Drug and Alcohol Department know if you are needing additional supplies or if you have not yet received your updated supplies.
